How to Apply For a Foundation Grant

To apply for a foundation grant on behalf of a hospital department, please complete the grant form online through the Norman Regional’s intranet- MyLink. Directions are as follows:

  • Open MyLink
  • Click the Resource tab
  • Under the Grants & Giving table click Foundation Grant Form
  • Follow the application instructions and fill out the application

Note:

You must also print a hard copy of the completed online grant application for leadership to approve and sign. Then, complete your submission by forwarding the signed application to the Foundation for final processing. Our office is located at the Porter campus.

Please deliver the form to the Foundation by deadlines: March 31 and September 30. If required, please allow time to collect signatures.
